SEEDS is excited to welcome new and returning students to the SEEDS Contemporary Ballet Program—a free dance program designed to empower Black and Brown youth through the art of ballet. This program fosters discipline, creativity, and cultural pride through structured dance education.
Who Can Join ::
Open to youth ages 3–20, the SEEDS Program provides:

Professional dance training

All dance attire provided (including leotards, tights, and ballet shoes)

Performance opportunities throughout the school year

A safe, supportive environment that builds confidence, expression, and technical skills

2025–2026 Season Registration ::
By completing the registration form below, families will secure a spot for their child in our upcoming 2025–2026 ballet season. The form collects essential student and guardian information, emergency contacts, health details, and attire sizes to ensure every dancer is fully supported.

Parents/Guardians: By signing this document, I agree:

To sign my child out on time. If I can’t do so, I will make arrangements with another individual listed on this form to pick up my child on time.

To notify staff of emergency changes in my child’s contact information.

To the expectations of children enrolled in SEEDS Nonprofit Org. programs (Yes, I have reviewed and will support them)

To authorize SEEDS Nonprofit Org. to act on my behalf if my child is injured or becomes ill and requires medical care beyond basic first aid, as long as SEEDS Nonprofit Org. staff makes a diligent effort to notify me.

That SEEDS Nonprofit Org., nor any person associated with them, has any responsibility to me or my youth from any claims arising from any accident, injury, or illness my youth may suffer due to any such healthcare or medical treatment.

To hold harmless SEEDS Nonprofit Org., all its staff, volunteers, other funders, and any affiliated organizations and entities from any liability or damages incurred as part of the conduct and execution of any prescribed activities or programs.

That SEEDS Nonprofit Org. may secure licensed medical professionals to care for my child if required.

That SEEDS Nonprofit Org. may transport my child on field trips during regularly scheduled program hours. Activities outside regularly scheduled hours will require additional permission.

To release photos, videos, recordings, etc., to SEEDS Nonprofit Org. and media approved by SEEDS Nonprofit Org. for use in publications, including internet-based and social media outlets.
